Geriatric Care Managers of WNY was founded out of an identified need in our community. Many WNY families are often overwhelmed and confused when it comes to dealing with the health and concerns for their elders. Families are often scattered all over the country or are so busy with their day to day lives that when that crisis hits they just don't know where to begin.
A Geriatric Care Manager is someone who is expert in navigating the current health care system and can be a valuable resource to both the elder and their families. Assisting you and your family in finding:

We will provide a comprehensive assessment of the entire situation-the person, the place, the health status, the documents, and the finances. And assist in locating the most appropriate rehabilitation setting, Assisted Living, Skilled Nursing facility or home, whatever the situation calls for.
Our Geriatric Care Managers are committed to empowering families and professionals with the knowledge and resources needed to assist elders in aging with dignity, independence and respect. We evaluate each clients needs, review available options and monitor the care once delivered.
