Assessment
- Determine individual needs and help locate appropriate services
- Provide a comprehensive health assessment, completed by a Registered Professional Nurse
- Provide an in home safety evaluation
- Provide a financial assessment to determine payor sources available for care and how to access additional sources of payment for services as needed
- Assess current documents.
- Work with your chosen physician to ensure the effectiveness of prescribed treatments
Evaluation
- Make recommendations for care options and settings
- Assist with placement issues and determine the most appropriate level of care necessary
- Facilitate family meetings to help with care decisions
- Provide communication and support for long-distance family members
- Assist with aging issues, crisis intervention, rehabilitation plans and many other health-related issues
- Work with community resources for social support
Facilitation of accessing additional Payor Sources
- Explore options of additional payor sources to assist in paying for the necessary care
- Assist in determine eligibility for Medicare, Long-term Care Insurance, Medicaid as well as VA benefits and other potential sources of reimbursement
Ongoing follow up
- Each care manager will develop an individualized system of follow up for each family, whether written, verbal or e-mail communication
