Assessment

  • Determine individual needs and help locate appropriate services

  • Provide a comprehensive health assessment, completed by a Registered Professional Nurse

  • Provide an in home safety evaluation

  • Provide a financial assessment to determine payor sources available for care and how to access additional sources of payment for services as needed

  • Assess current documents.

  • Work with your chosen physician to ensure the effectiveness of prescribed treatments


Evaluation

  • Make recommendations for care options and settings

  • Assist with placement issues and determine the most appropriate level of care necessary

  • Facilitate family meetings to help with care decisions

  • Provide communication and support for long-distance family members

  • Assist with aging issues, crisis intervention, rehabilitation plans and many other health-related issues

  • Work with community resources for social support


Facilitation of accessing additional Payor Sources

  • Explore options of additional payor sources to assist in paying for the necessary care

  • Assist in determine eligibility for Medicare, Long-term Care Insurance, Medicaid as well as VA benefits and other potential sources of reimbursement


Ongoing follow up

  • Each care manager will develop an individualized system of follow up for each family, whether written, verbal or e-mail communication
