Services

Our services include:
Initial Assessment

Geriatric Care Managers will review and assess your loved one’s health, home, documents and financial situation to determine their overall needs and any gaps in their care. We will:

  • Determine overall individual needs and necessary services
     

  • Provide a comprehensive health assessment, completed by a Registered Professional Nurse
     

  • Provide a review of current medications and work with your chosen physician to ensure compatibility and effectiveness of prescribed treatments
     

  • Provide an in-home safety evaluation
     

  • Provide a financial assessment to determine payor sources available for care and how to access additional sources of payment for services as needed
     

  • Assess current documents including long term care insurance, Health Care Proxy and Power of Attorney to determine if all is in order and according to your loved ones wishes.

 

Plan Development

Geriatric Care Managers will work with you and your family to develop a plan in the best interests of your loved one. We will prioritize actions, set an appropriate timeframe and outline responsibilities including:

  • Make recommendations for care options and settings
     

  • Assist with placement issues and determine the most appropriate level of care in the right setting
     

  • Facilitate family meetings to help with care decisions
     

  • Provide communication and support for family members, including those who live out of town
     

  • Assist with aging issues, crisis intervention, rehabilitation plans and many other health-related issues
     

  • Work with community resources for social support

 

Review Financial Resources

Many families are concerned with how to pay for necessary services for their loved ones. We will:

  • Explore options of additional payor sources to assist in paying for the necessary care
     

  • Assist in determining eligibility for Medicare, Long-term Care Insurance, Medicaid and Veterans Administration benefits and other potential sources of reimbursement
     

  • Work with your family to apply for and receive reimbursement for care

 

Ongoing Follow Up

Geriatric Care Managers will provide on-going support and follow up to your family for as long as necessary.  We will:

• Develop an individualized system of follow up for each family, whether written, verbal or e-mail communication

• Facilitate on-going checks on your loved one and the services they are using to ensure appropriate care
